PAYROLL PROTECTION PROGRAM - ROUND TWO

Required for submission:

1. Must be a Sole Proprietor, Independent Contractor or a Self Employed Individual.
2. Completed, signed, initialed SBA Form 2483-SD-C.

3. Supporting documents showing the applicants 25% or greater decrease in revenues from any calendar quarter in 2019 compared to the same quarter in 2020.  Applicants can also chose to demonstrate a decrease of 25% or greater for the year of 2020 compared to 2019.

4. 2019 & 2020 Profit & Loss Statement for each year.

5. Documents such as: Business Financial Statements, Profit & Loss Statement, Tax Returns, Sales Reports, etc.
